Long-term recurrence of bile duct stones after endoscopic papillary large balloon dilation with sphincterotomy: 4-year extended follow-up of a randomized trial.

In this paper, we investigated the long-term outcome of endoscopic papillary large balloon dilation (EPLBD) and endoscopic biliary sphincterotomy (EBS) in the treatment of CBD stone, 30s vs. 60s ballon dilation comparison trial) was followed by extended follow-up (4 years). Among other studies, CBD stone recurred in about 11% of long-term cases with EBS only. Prospective follow-up showed that the prevalence of previous cholecystectomy was 7.5% in 106 patients and 51% in patients who had received EBS before. Of these patients, 8 of 106 patients with a recurrent CBD stone showed a 7.5% dml ratio and the median baseline diameter was significantly longer than those without recurrence. Discussion of this is that sphincterotomy increases CBD and decreases sphincter tone and CBD angulation, which may lead to better stasis and thus stone recurrence . In addition, the greater the diameter of the CBD, the greater the risk of bacterial contamination. Therefore, it is suggested in the paper that the management of patients with EPLBD + EBS will be made easier by predicting the prognosis and the risk of stone recurrence according to the CBD diameter after procedure and determining the frequency of f / u .
