Cushing's syndrome: Symptoms & Causes

Causes
Cushing's syndrome is a syndrome characterized by hypertension, fatigability, weakness, sexual dysfunction, striae, edema, osteoporosis, and truncal obesity of chronic glucocorticoid (cortisol). Cushing's syndrome is the cause of exogenous origin of cushing's syndrome caused by drugs such as glucocorticoid. The next is cushing's syndrome of endogenous origin. ACUT-dependent pituitary adenoma, cushing's disease, is a microadenoma of less than 10 mm mostly over 90%. It may also be caused by ectopic ACTH secretion. This may be caused by SCLC, thymoma, pancreatic islet cell tumor, MTC, and pheochromocytoma, which induces hypersecretion of ACTH at the origin, not the pituitary gland.
ACTH independent causes are adrenal neoplasm and adrenal nodular hyperplasia. Adrenal neoplasm is usually unilateral, 8-10% of adenoma, and 8-10% of carcinoma. Therefore, it must be distinguished, and if it is uncertain as to the imaging test, postoperative biopsy is essential. It is also important to distinguish between the adrenal mass itself and the presence or absence of lymph node involvement or distant metastasis. Adrenal nodular hyperplasia macronodular, micronodular-sporadic, and familial.

Symptoms
Clinical features of Cushing's syndrome include moon face, buffalo hump, and truncal obesity due to centripetal obesity, which may be due to lipid redistribution. There is also a clinical phenomenon caused by the occurrence of the catabolic effect. Inhibition of Ca absorption in the kidneys leads to hypercalciuria, decreased Ca absorption in the intestine, inhibition of cartilage and bone formation, resulting in osteoporosis, weakness of the dermis collagen, disorder of rupture, easy bruisability and red striae have. Furthermore, elevated renin levels in the liver due to cotisol elevation and elevated angiotensin + hypertension due to mineral corticoid effects of cortisol itself. The rise of adrenal androgens can also result in masculinizing symptoms. In addition, psychiatric symptoms such as depression or psychosis can occur in multiple systems and can cause many symptoms.
